Starting in July 2025, the PG office is moving the administration of the AVP detailed assessment form (Detailed AVP Form for IMGs) to a digital format using Elentra. You are responsible for submitting the Detailed AVP Form for IMGs at the designated intervals (4, 8 and 12 weeks) with additional assessments if needed.
The Detailed AVP Form for IMGs has been built to match the paper form and must not be modified by any program.
When you need to send a Detailed AVP Form for IMGs to an assessor, please complete the following steps.
- Go to Admin > Assessment & Evaluation.
- Assuming you have multiple tasks to send out, click Trigger Bulk Assessment.
Note: If you have only one task to send, you can use the Trigger Assessments option. - On the Generic Assessment Tools screen:
- Select Program
- Browse Assessors: Add the name of any assessor you need to complete the AVP
- Browse Learners: Select a curriculum period and then search for the relevant trainee and check the radio button by their name(s)
- Enter the relevant start and end dates for the task (e.g. beginning and end of 4-week period). Note, these dates can be unique per trainee.
- Use the Add Assessor option if you’d like to an add another assessor and trainees
- Check off “These assessments are mandatory” and “Prompted Response Notifications”
- Under Users to notify check off Program Directors and Custom Reviewers; under Custom Reviewers add Laura McEwen and Leslie Barroso.
- Click Get Tools.
- Search for AVP and look for Detailed AVP Form for IMGs.
- Click Send Assessment.
- Optionally enter an Assessment Cue. This will display at the top of the task for the assessor and will also be visible to the trainee once the task is complete.
- Click Send Assessment.
- The task will send to the assessor immediately.
Tracking Task Completion
If you wish to check on the status of the AVP tasks you have sent you, follow these steps:
- Go to Admin > Assessment & Evaluation.
- You will land on the Outstanding Tab.
- Search for Detailed AVP Form for IMGs
- Any outstanding (i.e., to be completed) forms should display. The ‘owner’ is the person who needs to do the task. You can check off the reminder column for any tasks that you want to remind people about.
If you want to quickly see all completed AVP tasks, switch to the Completed tab and perform the same search.
You can also review AVP tasks by visiting an individual learner's Assessments page.
Additional Notes:
- This form is not available on demand for trainees or faculty to trigger. It must be sent out by a PA. This is to ensure the correct people are using the form at the correct time(s).
- If it is better for your program to send these tasks via distribution (e.g., a delegation to PA or something), that's fine. The instructions above are just the simplest path in Elentra to send these tasks.
- All items on the Detailed AVP Form for IMGs include prompted response notifications for any Unsatisfactory, Below Expectations or N/A or Unable to Assess response.
- The Final AVP form will be emailed to your program by the PGME Office. If you don’t receive a copy, please contact PGME.
- If a trainee has below expectations, unsatisfactory, or unable to assess, all detailed forms will need to be provided in one link or hardcopy to the PGME Office.
